Wilson Disease Overview

Wilson disease is an autosomal recessive disorder caused by pathogenic variants in ATP7B, which encodes a copper-transporting ATPase required for biliary copper excretion. Impaired ATP7B function causes copper accumulation in the liver and, with progression, the brain, cornea, and other tissues. Clinical manifestations vary and may include hepatitis, cirrhosis, acute liver failure, tremor, dystonia, dysarthria, movement disorders, psychiatric symptoms, and Kayser–Fleischer rings. Treatment aims to remove excess copper and prevent further accumulation using penicillamine, trientine, and/or zinc. Liver transplantation is reserved for selected patients with acute liver failure or advanced liver disease. Despite effective therapies, lifelong treatment, adherence demands and persistent neurological symptoms remain challenges.
