This deficiency manifests as a pyridoxal 5'-phosphate-responsive epileptic encephalopathy, with refractory seizures emerging typically within the first year of life. Lifelong treatment with B6 vitamers, primarily pyridoxal 5'-phosphate (PLP) in 60% of cases and pyridoxine (PN) in 40%, yields partial seizure control. Developmental impairment, affecting speech, cognition, and behavior, is present in around 60% of individuals with PNPO deficiency. Some may also exhibit neurological deficits such as muscular hypotonia or dystonia. Severe neurodevelopmental impairment is more prevalent in cases with delayed diagnosis and prolonged periods of uncontrolled seizures. Pharmacological management involves oral administration of PLP or PN, with PLP typically divided into 4-6 daily doses and PN into 3-4 doses. Careful titration is essential to avoid potential liver toxicity with PLP and sensory or motor neuropathy with PN, which is usually reversible upon dose reduction.
