Oropharyngeal Squamous Cell Carcinoma Overview
Oropharyngeal squamous cell carcinoma (OPSCC) is a malignancy arising from the oropharynx, most commonly involving the palatine or lingual tonsils and base of the tongue. Human papillomavirus (HPV), particularly HPV-16, is an important etiologic factor in a substantial proportion of cases, while tobacco and alcohol exposure remain major risk factors, especially for HPV-negative disease. Clinical presentation may include a persistent sore throat, difficulty or pain with swallowing, ear pain, voice changes, or a neck mass caused by cervical lymph-node involvement. Treatment is determined by tumor stage, HPV status, anatomical site, and patient factors and may include surgery, radiation therapy, concurrent chemoradiation, systemic therapy, or combinations of these approaches. Although HPV-positive disease generally has more favorable outcomes, treatment can produce substantial long-term effects on swallowing, speech, salivary function, and quality of life. Current research is therefore focused on treatment de-intensification, organ preservation, immunotherapy, biomarker-guided strategies, and approaches that maintain disease control while reducing treatment-related morbidity.
