Myeloproliferative Neoplasms (MPNs) Overview
Myeloproliferative neoplasms (MPNs) are a group of rare, chronic hematologic malignancies characterized by the clonal expansion of hematopoietic stem cells in the bone marrow, resulting in overproduction of one or more mature blood cell lines, including red blood cells, white blood cells, and platelets. The three “classic” BCR‑ABL1–negative MPNs are polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF). These disorders share common driver mutations in JAK2, CALR, or MPL, which lead to constitutive activation of the JAK‑STAT signaling pathway. Clinically, MPNs are associated with an increased risk of thrombotic and hemorrhagic complications, as well as splenomegaly and constitutional symptoms. Over time, they may progress to advanced disease or transform into acute myeloid leukemia (AML). Management is risk‑stratified and focuses on preventing vascular events, controlling blood counts, and alleviating symptoms through phlebotomy, low‑dose aspirin, cytoreductive therapies, JAK inhibitors, and, in selected cases, allogeneic stem cell transplantation.
