Leber’s Hereditary Optic Neuropathy (LHON) is a maternally inherited mitochondrial disorder characterized by subacute, painless central vision loss, typically beginning in one eye and followed by involvement of the second eye within weeks to months. It predominantly affects males in their teens or twenties, although onset can occur at any age. The acute phase presents with blurring, reduced color perception, and central or cecocentral scotomas, often accompanied by fundoscopic findings such as peripapillary telangiectatic vessels and optic disc hyperemia. Optic atrophy generally develops within a few months, marking the transition to the atrophic phase, which results in bilateral optic nerve pallor and permanent visual impairment, often worse than 20/200. LHON is caused by point mutations in mitochondrial DNA—most commonly m.11778G>A, m.14484T>C, and m.3460G>A—affecting respiratory chain complex I. Although maternal transmission is strict, penetrance is incomplete and influenced by environmental and nuclear genetic factors. Rarely, LHON may present with systemic features such as peripheral neuropathy, tremor, cardiac conduction defects, or multiple sclerosis-like syndromes, collectively referred to as “LHON Plus.”
