Glanzmann Thrombasthenia (GT) Overview

Glanzmann thrombasthenia (GT) is a rare autosomal recessive bleeding disorder caused by ITGA2B or ITGB3 mutations. These defects impair the platelet surface glycoprotein (GP) IIb/IIIa integrin complex, preventing fibrinogen binding and platelet aggregation. Patients present with severe mucocutaneous bleeding (epistaxis, menorrhagia, surgical hemorrhage). The diagnostic hallmark is classic: despite normal platelet counts and morphology, aggregometry shows an absent response to ADP, collagen, and epinephrine, but preserved agglutination to ristocetin. Diagnosis is confirmed via flow cytometry showing reduced/absent GPIIb/IIIa and genetic sequencing. Management centers on controlling acute bleeds. Platelet transfusions are standard but carry a high risk of isoimmunization (forming antibodies against the missing GPIIb/IIIa), which renders future transfusions ineffective. For alloimmunized patients, recombinant activated factor VII (rFVIIa) is the hemostatic standard. Antifibrinolytics (tranexamic acid) serve as vital mucosal adjuncts. While curative hematopoietic stem cell transplantation (HSCT) is reserved for severe cases, careful transfusion stewardship ensures a favorable prognosis.
