Facioscapulohumeral Muscular Dystrophy Overview

Facioscapulohumeral muscular dystrophy (FSHD) is an inherited myopathy in which epigenetic derepression of DUX4 in skeletal muscle causes toxic gene expression and muscle injury. The disease produces slowly progressive, asymmetric weakness of the facial, scapular, humeral, trunk, and lower-limb muscles, with variable severity and progression. Respiratory involvement can occur. There is still no approved disease-modifying therapy; management focuses on physical therapy, exercise, pain control, respiratory monitoring, orthotic support, and orthopedic interventions such as scapular fixation. Therapeutic development is shifting toward direct DUX4 suppression. Delpacibart braxlosiran (AOC 1020), an antibody-oligonucleotide conjugate delivering DUX4-targeting siRNA to skeletal muscle, is in Phase 3 development after encouraging biomarker results. ARO-DUX4, another DUX4-targeting siRNA, is in clinical development. Durable DUX4 suppression and preservation of muscle function remain main goals.
