X-Linked Retinoschisis (XLRS) Overview

X-linked retinoschisis (XLRS) is a hereditary retinal dystrophy resulting from mutations in the RS1 gene located on the X chromosome at position Xp22.1. This genetic alteration leads to schisis (splitting) of the neural retina, primarily affecting men. The condition accounts for nearly all cases of congenital retinoschisis, with occasional reports of autosomal dominant retinoschisis comprising the remaining cases. Importantly, retinoschisis differs significantly from retinal detachment, as it involves splitting within the inner retinal layers rather than between the neural retina and the retinal pigment epithelium. Disease severity varies widely, even among individuals harboring the same causative RS1 mutation. Interestingly, there is no discernible correlation between the specific mutation type and the severity or progression of the disorder. Typically, patients present with poor vision and reading difficulties during school age, although some may exhibit symptoms as early as 3 months of age. The age of onset follows a bimodal distribution: some patients present in infancy with squint and nystagmus, while others with only poor vision become apparent during school age. Visual impairment shows marked variability, with best-corrected visual acuity ranging from 20/20 to 20/600. These differences persist across all ages, even within families or among patients with identical mutations. Foveal schisis, characterized by a cartwheel pattern of folds radiating outward from the fovea, is a hallmark feature of XLRS and is present in 98–100% of cases. However, this pattern may become less distinct over time. Additionally, peripheral retinoschisis is often observed in the inferotemporal region. During infancy, these cavities may manifest as large bullous retinoschisis, which tends to regress, leaving behind lines of pigment in older patients. The diagnosis of XLRS is established in male individuals with suggestive ophthalmologic findings and the identification of a hemizygous pathogenic variant in the RS1 gene through molecular genetic testing. Management strategies include addressing refractive errors and amblyopia according to standard care guidelines. Ambylopia prevention therapy is recommended following surgical intervention for vitreous hemorrhage, retinal detachment, or severe retinoschisis. Low-vision aids, such as large-print textbooks, preferential seating in classrooms, and high-contrast handouts, can enhance visual function. In some cases, surgery may be necessary to manage infrequent complications like vitreous hemorrhage or full-thickness retinal detachment.
