Ventilator-Induced Lung Injury (VILI) Overview

Ventilator-Induced Lung Injury (VILI) could occur during invasive and non-invasive ventilation and could significantly contribute to the morbidity and mortality of critically ill patients. Though mechanical ventilation can injure both normal and diseased lungs, the latter will suffer far more severe damage due to higher microscale stresses. The terms ventilator-induced lung injury (VILI) and ventilator-associated lung injury (VALI) have been used interchangeably (VALI). The most common mechanisms of ventilator-induced lung injury are alveolar overdistention (volutrauma), barotrauma, atelectotrauma, and inflammation (biotrauma). Other attributed mechanisms include adverse heart-lung interactions, deflation-related, and effort-induced injuries. Even though both strategies result in identical high levels of end-inspiratory pressure and alveolar distension, ventilation with high tidal volume and low or zero PEEP appears to be more harmful than ventilation with low tidal volume and high PEEP. Ventilation of isolated lavaged rat lungs with small tidal volumes (5–6 ml/kg) and low or zero PEEP caused lung injury that increasing PEEP levels could mitigate.
