Thyroid eye disease (TED) is an autoimmune disorder characterized by inflammation of orbital tissues. Various grading systems, such as the European Group on Graves Orbitopathy (EUGOGO) protocol, VISION classification (assessing vision, inflammation, strabismus, and appearance), NOSPECS classification, and Mourits system, aid in determining TED severity and tracking disease activity. Due to the diverse presentations, tailored clinical evaluation and ancillary testing are necessary. TED can mimic other conditions, including orbital tumors, myasthenia gravis, and systemic inflammatory diseases, making accurate diagnosis challenging. Teprotumumab (Tepezza), an FDA-approved immunotherapy, targets the insulin-like growth factor-1 receptor (IGF-1R) on orbital fibroblasts, inhibiting their activation and signaling.

  • In the United States, TED incidence is higher in women, with an annual rate of 16 per 100,000 person-years compared to 3 per 100,000 person-years in men. The highest prevalence occurs in individuals aged 50–54 years, with women consistently exhibiting a threefold higher prevalence rate across different age groups and racial/ethnic backgrounds.
