The pathophysiology of metastatic bladder cancer is intricate and encompasses numerous factors. The pivotal events in this process contain genetic changes enabling cancer cells to proliferate uncontrollably, the secretion of factors stimulating the growth of blood vessels (angiogenesis), and the capability of cancer cells to infiltrate and spread to other body regions. Several factors increase the risk of metastatic bladder cancer - High-grade tumor: Tumors with a high grade are more prone to metastasis, Muscle-invasive tumor- Tumors that have penetrated the muscle layer of the bladder wall are at a higher risk of metastasis, Distant metastases at diagnosis- If a patient presents with distant metastases upon diagnosis, it signifies that the cancer has already disseminated to other body parts, Specific genetic mutations- Certain genetic mutations, such as those in the TP53 gene, are linked to an elevated risk of metastasis, Advanced age- Older adults are more susceptible to developing metastatic bladder cancer. Chemotherapy is a systemic treatment approach commonly involving a combination of cisplatin drugs. Immune checkpoint inhibitors like pembrolizumab, durvalumab, atezolizumab, and avelumab (Bavencio) are used to manage bladder cancer. Enfortumab vedotin (Padcev) is a monoclonal antibody utilized to treat metastatic bladder cancer. For cases of metastatic bladder cancer with mutations in the FGFR2 or FGFR3 gene that are unresponsive to chemotherapy, erdafitinib (Balversa) can be employed. Metastatic bladder cancer can give rise to various complications, contingent on the locations in which the cancer has spread. Complications associated with advanced bladder cancer may encompass erectile dysfunction, anemia, urinary incontinence, urinary tract infections (UTIs), or inflammation.
