Primary hyperoxaluria is a rare inherited metabolic disorder, most commonly type 1, in which a defective liver enzyme causes excessive oxalate production. This leads to recurrent kidney stones, progressive calcium oxalate deposition, declining kidney function, and, in advanced disease, systemic oxalosis affecting multiple organs. Historically, patients with kidney failure often required combined liver–kidney transplantation because replacing the liver was the only way to correct the underlying metabolic defect.
