Fibrodysplasia ossificans progressiva (FOP) is a condition in which skeletal muscle and connective tissue, such as tendons and ligaments, gradually deteriorate and are replaced by bone. This condition causes bone formation outside the skeleton, limiting movement. Fibrodysplasia ossificans progressiva (FOP) is characterized by congenital bilateral hallux valgus malformations and early-onset heterotopic ossification, which may be spontaneous or precipitated by trauma, including intramuscular vaccinations. Painful, recurrent soft-tissue swellings (flare-ups) may precede localized heterotopic ossification. Heterotopic ossification can occur at any location but typically affects regions near the axial skeleton in the early/mild stages before progressing to the appendicular skeleton. This process generally becomes noticeable in early childhood, beginning with the neck and shoulders and progressing down the body and into the limbs. FOP patients have abnormal big toes (hallux valgus), which can aid in diagnosis. Trauma, such as a fall, invasive medical procedure, or a viral illness, may trigger muscle swelling and inflammation episodes. The diagnosis of FOP is established in a proband with heterotopic ossification, hallux valgus malformations, and a heterozygous pathogenic variant in ACVR1 identified by molecular genetic testing.
