Atrioventricular Septal Defect (AVSD) Overview

Atrioventricular septal defects (AVSD) are congenital heart conditions marked by varying degrees of atrial and ventricular septal defects and a shared or partially separated atrioventricular orifice. A partial AVSD involves specific features like an atrial septal defect, separate atrioventricular valves with a common junction, a ventricular septal defect, and a cleft mitral valve. On the other hand, a complete AVSD features a common atrioventricular valve with an atrial septal defect and an unrestricted inlet-type ventricular septal defect. The clinical presentation of AVSD depends on the type of defect, the size of the intracardiac shunt, and any accompanying heart malformations. Complete AVSD patients often show signs of pulmonary congestion and right heart failure early in life due to a substantial left-to-right shunt, significantly as pulmonary vascular resistance decreases after birth. Heart failure and Eisenmenger syndrome might develop earlier in cases with atrioventricular valve regurgitation, ventricular imbalance, or aortic coarctation. Patients with partial AVSD, without complex congenital heart issues and minimal valve regurgitation, usually remain asymptomatic in early childhood and are typically diagnosed incidentally based on findings like heart murmurs and fixed splitting due to an atrial septal defect. The prognosis for untreated AVSD is bleak, with roughly 50% of patients not surviving infancy due to heart failure or pulmonary infections. Those who survive beyond the first year often develop irreversible pulmonary vascular disease and eventually, experience a reversal of the shunt. Patients who undergo surgical repair have a 90% survival rate for around 15 years, although approximately 9% to 10% of them may require reoperation within that period. Most complications related to AVSD are linked to intracardiac shunting and atrioventricular valve regurgitation. Complete AVSD leads to right-sided heart overload, heart failure, and pulmonary congestion at a young age, contributing to a significant infant mortality rate. This shunt can lead to irreversible pulmonary vascular disease, pulmonary hypertension, and Eisenmenger syndrome without correction.
